Who is eligible to receive services?
A resident of one of the six counties served (Fort Bend, Austin, Waller, Colorado, Wharton, Matagorda) who:
 • Has been diagnosed with Schizophrenia
 • Or has been diagnosed with Major Depression
 • Or has been diagnosed with Bipolar Disorder
 • And is at least 18 years of age.
How do I get help?
Individuals seeking treatment should contact the nearest Behavioral Healthcare Outpatient Clinic to make an appointment for an assessment/eligibility determination. In order to be seen, you will need to bring the following:
 • Medicaid card, Medicare card, private insurance card
 • Driver's license
 • Social security card
 • Proof of income (IRS 1040 or 1040 EZ form, paycheck stubs, etc.)
 • Number of individuals in the household
 • Utility bill showing that you live in one of the six counties that we serve
 • Records of any unusual expenses
If you do not bring these items, you will be asked to reschedule.
