The person seeking services, or someone who best knows the person seeking services, may make this call. The Texana Center Intake worker will ask a series of screening questions to determine if the person may qualify for intellectual and developmental disability services. The Intake worker will also ask questions to determine what services the person needs, and if Texana Center can help the person to receive these services.
Prior to the Intake phone call, the caller may gather information to have on hand for the Intake call and review a list of screening questions. During the Intake phone call, the Intake worker will explain the community services we offer and how we charge for services.
Following the Intake phone call, the Intake worker will mail or email an intake packet to the person. The packet includes forms to be completed by the person and will be used in the Eligibility process. Intake may be done in person at the Authority Services office location in Rosenberg.
