Service Coordination is assistance enrolling into programs, and assistance with planning and coordinating services after enrollment. This is a required service in most programs, meaning that the State requires that people have a designated Service Coordinator, who is employed by Texana Center, to help with enrollment into programs, help with planning the services they need, and help monitoring that the services make improvements in their lives. Service Coordinators also help people access services to prevent and manage crisis situations.
At Texana Center, Service Coordination is organized by the following program units:
Service Coordination and supports for people accessing and/or receiving personal assistance service and habilitation training. More Information, Contact Information for Service Coordinators, Contact Information for Managed Care Organizations (link to website page)
Service Coordination for people receiving services in various Texana Center programs funded by state funds. More Information, Contact Information for Service Coordinators
