Community First Choice (CFC) is a Medicaid program that provides basic attendant and habilitation services for individuals who require assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) to reside successfully in a community setting.
Services that are available include:
Provides assistance with self-care, managing health care, maintaining their home, and/or assisting families with caring for the person in their home.
Available to help train people on the skills they need to live independently in the community such as creating and following a budget, payment of bills, accessing transportation, maintaining their health, preparing meals and taking care of their home.
This service provides an electronic device worn by the person that, when activated, alerts a designated person that they are experiencing an emergency. This device is provided to people who live alone, are alone for long periods during the day, or do not have regular caregivers for extended periods of time.
This service provides training to the person on selecting, managing and dismissing attendants.
