All the services you need.

Services are provided in home or community settings and help children under age 3 learn basic skills as part of their family’s daily routines.

Service Coordination to ensure access to programs and community resources that are needed to help a family to reach their goals for themselves and their child
Screening and assessments to identify the child’s strengths and needs and family concerns
Individualized family service plans to address the family’s strengths and needs
Family support and education to help the child develop through daily routines and activities

Services are provided in home and community settings where the child spends time (such as child care facilities, play groups and Mothers’ Day Out programs)
Activities to help social and emotional development including behavior
Materials or equipment to help the child participate in daily routines and activities
Physical, occupational, speech and language therapy and activities related to motor and language development
Specialized Skills Training by an Early Intervention Specialist

Transition Services to assist the family and child to move on to school or other services at age 3 years old.
Healthcare support to include vision, hearing and nutritional screenings or services
Social work and counseling services to address child/parent relationships
Family events and parent education programs
Access to identified support groups or community resources
